Laboratory Services Patient Information

Special instructions for the patient.

In order to provide you with an excellent laboratory experience, we need your assistance with the following:

Please arrive at least 20 to 30 minutes before the posted closing time.  Patients are served in the order of arrival.

Bring the lab orders your DuPage Medical Group physician gave you for your testing.  If your DMG physician’s office plans to send the orders to the lab prior to your arrival, be sure to call the lab site where you plan to go to verify that the orders are waiting for you.  This will give the lab an opportunity to obtain the necessary information prior to your arrival.

You must bring your insurance card on the day of service.

Ask your doctor if you need to fast prior to your test.

Note: If you are to have a Gestational Diabetes Evaluation, be sure to arrive at least 2 hours prior to the posted closing time for the lab site you have chosen.
